NDIS Basics

The National Disability Insurance Scheme (NDIS) is a government-funded program in Australia that provides support and funding to people with disabilities. It’s designed to help improve independence, well-being, and quality of life by funding necessary services and supports. 

To be eligible for the NDIS, you must: 

  • Be under age 65 
  • Be an Australian citizen, permanent resident, or hold a protected special category visa 
  • Have a significant and likely permanent disability that substantially affects your ability to participate in daily life 

Services & Funding

Through NDIS funding, you may access: 

  • Occupational Therapy 
  • Speech Pathology 
  • psyc 
  • Behavioural Support 
  • Other allied health supports, depending on your individual plan 

No referral from a doctor is required. You can contact Chrome Health directly to arrange services, using the funding already outlined in your NDIS plan.

Treatment Planning & IAP

We use a collaborative Intervention & Action Plan (IAP)—a practical plan that translates your NDIS goals into clear therapy objectives, strategies, and review points.

What’s inside an IAP?
  • Goals & Outcomes: your NDIS goals mapped to therapy objectives (short- and medium-term).
  • Interventions: environmental modifications, skill-building, sensory strategies, coaching.
  • Roles & Frequency: who’s involved and how often sessions occur.
  • Measures & Evidence: functional assessments, observations, goal-attainment scaling.
  • Timeframes & Reviews: typical quarterly check-ins (or sooner if needed).
Quarterly benefits for clients:

Getting Started

  1. Reach out to us – we’ll talk through your NDIS goals and funding 
  2. Initial consultation – we get to know your strengths, needs, and vision 
  3. Personalised care plan – together, we co-design a plan that aligns with your identity and goals 
  4. Begin therapy – mobile or telehealth, whichever suits your comfort and accessibility needs 
  5. Ongoing support – we periodically review and adapt your plan in collaboration with you and your support network 

Coordination & Collaboration

Can Chrome Health coordinate with support coordinators?

Absolutely. We work collaboratively with support coordinators and other members of your support network to ensure your care aligns with your NDIS plan and goals. 

Admin & Changes

We’re flexible. If your situation changes, or you need to reschedule or cancel, please let us knowideally in advance, so we can manage your support effectively.

How can DCJ refer?

We accept referrals directly from the Department of Communities and Justice. Our team works closely with caseworkers, guardians, and support coordinators to ensure individuals receive timely and appropriate care.
